Observation

WAC 110-300-0165(2)(c): At the time of the site visit there was a hanging cord from the wall hand dryer in the bathroom that formed a loop and a cord hanging down and forming a loop from a singing box located on the office desk in the back bedroom where children have been napping and both cords are accessible to the children. WAC 110-300-0165(2)(d): At the time of the site visit there was a plastic garbage bag located in the lower white cabinet in the bathroom and two plastic bags located in a plastic drawer that was sitting on top of the lower white cabinet of the bathroom. The provider removed the plastic bags at the time of the site visit. WAC 110-300-0165(3)(f): There is a large mirror in the back bedroom where the children have been napping that is about 24 inches wide and the height up to this licensors neck and it is leaning up against the wall and is a risk of falling on children who may play or walk around it. There is also a pile of miscellaneous stuff in the open closet of the back bedroom and in front of the closet and out from the closet that can be a falling hazard. WAC 110-300-0165(3)(g):At the time of the site visit there was a round table that was painted grey and there were multiple places where the paint was flaking off (some spots the size of a quarter). WAC 110-300-0165(4)(f): At the time of the site visit there was a toddler and an infant that were crawling on the stair way.
